At Wareham Pediatrics, we strive to provide the best possible care to our adolescent patients. In order to provide high quality care to your child, it is essential that they feel free to share any information with the expectation that it will be kept private. Our late teen/young adult patients have a choice, they can continue to receive their care in our office or transition to an adult care provider.  WPA will assist you in this decision.  If you wish, you can inform us of this decision by using this form.
